e*volve Therapeutic Center accepts multiple insurance plans. If your plan is not listed below, contact your insurance company to see if they will reimburse for out of network expenses.
Insurance benefits will be verified prior to the first appointment. Payment for deductibles and co-payments is due at time of service unless other arrangements have been made.

TELEPHONE AND EMAIL COMMUNICATION FEES
Lengthy telephone calls and sometimes emails will be billed. The client will be responsible for any unpaid portion of that service.
CANCELLATION FEES
If an appointment is not kept and not canceled at least 24 hours prior to the scheduled time, a $75 fee will be charged to the individual.
